The Assistant Director, Clinical Services for Housing Community, RN, assists in providing guidance and leadership to the clinical operations for Assisted Living, Enriched Living and Housing functions.
With the Director, the Assistant Director plans, organizes, directs, coordinates, and evaluates services and activities provided by the licensed Home Care Agency. In collaboration with the Director, is responsible for the administration, quality improvement, professional, clinical and financial operations of the agency to assure a safe, effective and efficient program. Develops, communicates and implements policy and procedure consistent with corporate guidelines to assure adherence to all pertinent regulations and codes.

The Manager, ALP Home Care RN is responsible for ensuring safe, effective and efficient clinical care is provided by the clinical care team consisting of RN’s, LPN’s, physical therapists, occupational therapists, speech therapists, care managers (social workers) and aides in an Assisted Living Program. Provides support for aide supervision. Assesses, plans, implements, and evaluates effectiveness of nursing care delivered and communicates information appropriately to physicians and the interdisciplinary team. Assures adherence to corporate policies, procedures and all regulatory guidelines for clinical staff.

Ability to follow instructions and work as part of a team, performing tasks such as floor maintenance, apartment preparation, and very basic non-skilled maintenance. Maintain area in a clean and sanitary fashion as directed by a supervisor. Duties may include either housekeeping and/or dining services. All other duties as detailed will be explained by the department manager.

Under the supervision of the Transportation Coordinator, is responsible for the direct transportation of participants who may be physically or mentally limited to and from their destinations. Position requires constant interaction with participants, participant’s family and the staff of the Interdisciplinary Team.

The Desktop Support person will provide first-level daily support and problem determination for online services, workstation platform software and hardware, client/server applications, TCP/IP-based LAN and WAN networks, and organizing and directing the analysis and resolution of online problem situations through a problem management system (TrackIT) in a primarily Microsoft Windows-based environment to ensure optimal client satisfaction with their technical environment. The person will also troubleshoot problem areas (in person, by telephone, via e-mail, or remote login) in a timely and accurate fashion, and provide end-user assistance where required with both hardware and software issues. The Senior Client Support Analyst is a mentor and provides direction to the Desktop Support person and Associate Client Support Analyst. The candidate will assist in administration of Help Desk tools, will proactively share information within and across groups, and will provide accurate and meaningful documentation of problem resolutions.

Under the supervision of the Clinic Manager and general direction of the Administrator/Director of Clinical Operations, the Clinic LPN is responsible for management of the medication program for the D&TC and PACE participants, including reordering of participants medications from pharmacy; conducting a variety of quality assurance functions per guidelines to monitor quality of services provided; completing variance reports as necessary; administering medications as ordered during day center hours of operation; and working closely with pharmacy personnel to ensure medication deliveries are accurate and timely. The Clinic LPN is also responsible for performing the highest quality of patient care and ensuring patient safety at all times according to accepted nursing practice standards.
