Overview

The Supervisor, Enriched Living LPN provides leadership for aide activities during shift. In cooperation with other team members, assure that residents/clients are provided therapeutic and rehabilitative measures to maximize independence. Monitor and manage staff to provide assistance to clients/residents with personal hygiene. Monitor adherence to all policy and procedure to assure compliance with DOH and all other pertinent regulations.

 

Responsibilities

  • Monitors staffing activities to assure adequate coverage with optimal use of overtime.
  • Upon new admission/re-admission, meets with residents and sets up personal care schedules and needs.
  • Collaborates with Clinical team on Admissions/re-admissions. Reviews Medical Evaluation and clarifies discrepancies.
  • Assures effective communications between direct care staff and client/resident families.
  • Assures resident/client supplies are ordered in a timely manner.
  • Collaborates with Clinical team regarding progress on hospitalized residents. Prepares for discharges.
  • Assures that all required medications are ordered in a timely manner. Audits medication cart, narcotics weekly.
  • Assures that a monthly staff schedule is posted according to published time frames, minimizing overtime while maintaining appropriate coverage.
  • Assists with Family meetings.
  • Interprets policy/procedure/regulations for staff to maintain compliance.
  • Collaborates with Clinical Care Supervisor to assure adherence to policy and procedure.
  • Monitors all Medication documentation, Administrator Records, and Task Sheets to assure accuracy, timeliness and completeness. Addresses deficiencies appropriately.
  • Manages physician’s medication change orders. Assures follow-up documentation is complete. Obtains all required signatures.
  • Performs periodic review of the medical evaluations (Including DSS3122, or 4449) with the Clinical Care Supervisor for accuracy.
  • Directs and assists as required to facilitate completion of documentation, such as MARs, 24-hour report, Incident Reports and Task Sheets.
  • Verifies Aide care plans are current and accurate. Notifies Clinical Supervisor of discrepancies.
  • Assists with data collection for all incident reports and inputs Reports as they occur.
  • Maintains monthly weight and weight book accurately and timely.
  • Assists employees in resolving payroll issues. Reviews/verifies time sheet as required.
  • Manages and facilitates orientation process for all new aide employees on shift.
  • Monitors and manages direct care staff effectiveness. Maintains staff development plans addressing deficiencies, including hiring, disciplines and terminations as required. Maintains confidentiality.
  • Conducts periodic staff meetings to provide information regarding policy and procedure, resident/client care issues, and other facility program related communications.
  • Monitors delivery of resident/client care to assure quality expectations are met. Maintains an effective resident complaint/issue resolution system with Clinical Supervisor for areas of responsibility.
  • Monitors adherence to care plans through rounds/ QA audits.
  • Assists in resolution strategies and implementation of plans to address issues/problems as they arise.
  • Follows and complies with Department of Health regulations
  • Monitors budget lines for staffing and supplies. Intitate remedial actions are required.
  • Assists Administrator in preparation of annual budget.

Qualifications

  • Current New York State license in good standing as Practical Nurse or equivalent experience.
  • 2-4 years recent experience in provision of services to acute/long term frail elderly.
  • 2-3 years supervisory experience in related field, preferably a union environment.
  • Familiarity with medical transcriptions.
  • Flexible schedule.
  • Knowledge of long term care regulations.
  • Outstanding communication skills
