Coordinate clinical practice of staff and follow up on areas of concern to assure wholistic care is provided to each resident. Serve as liaison between resident, family, physician, and Loretto staff. Provide daily assessment of each resident and communicate changing needs to appropriate personnel.
Provide direct input into resident care plan on a daily basis, communicating change in needs to interdisciplinary team. Participate in discharge planning activities. Assess residents abilities and suggest interventions to maximize independence and quality of life. Provide health training to residents, family and care givers as required. Assume duties of Nurse Manger in their absence. Assure completion and accuracy of 24 hour reports. Complete MDS/PRI documentation accurately and timely. Monitor medication errors and take action to minimize occurrences. Report to Nurse Manager for follow up.
Degree from an accredited school of nursing or college. Current NYS registration in good standing. (1-2) years experience in long term/acute care setting. (2) years minimum experience in general nursing practice. Proven assessment capabilities. Familiarity with NYS DOH regulations. Demonstrated supervisory skills.
