Provide comprehensive social work services to program participants. This includes but is not limited to:
psychosocial evaluation, assessment for admission, patient's rights and advocacy, social case work, group
work and family counseling as appropriate. Key member of interdisciplinary team.
Maintains confidentiality - to meet HIPPA regulations.
Attends all admission interdisciplinary team meetings.
Assists in the development and implementation of social work policies and procedures in
collaboration with other team members and the Quality Assurance lead.
Effectively supports corporate goals and initiatives through personal goal setting and monitoring progress.
Assures all documentation is accurate, timely and meets all regulatory requirements.
Maintains required documentation regarding participant care and progress.
Supervise BSW and MSW students as appropriate.
Maintains current knowledge of community resources and new applicable regulations.
Educates other team members regarding community resources, Medicaid and other social programs and psychosocial issues related to participant care.
Counsels and instructs paraprofessional staff in psycho-social and commuication issues and assists with difficult family/care giver conflicts.
Participates in the development and revision of the Plan of Care as a member of the

Presents social work concerns and issues, suggestions for intervention and recommendations for revised treatment plan.
Review significant events/occurences as related to family, finance, and psycho-social behavior changes with interdisciplinary team.
Intervenes in crisis, attending to the emergency needs of the participant and/or family relating to illness, disability, deterioration of independence, etc.
Assists participant/care givers in making necessary applications for public assistance, housing placement, pre-admission screening, etc.

Intergrates the social work treatment plan into the overall plan of care developed by the

Determines participant and family needs related to social support, financial support or
counseling, housing appropriateness and psychological supports.
Maintains communication with other team members to discuss progress and coordinates

- Master of Social Work degree from an accredited program.
- Certification by NYS Education Department as a Social Worker
Minimum (1) year experience as a professional case worker with geriatric care, long term care or community services to the aging in a direct service capacity.
