  • Overview

    The Admission Discharge Nurse (A.D.N.) shall assess, plan, implement, and evaluate the effectiveness of nursing care delivered to the patients. They shall adhere to the established policy and procedures and the systemic clinical changes. The A.D.N. will be responsible for overseeing and completing the required assessments and documentation for the effective management of resident’s covered under the Bundled Payments for Care Improvement (BPCI) and will monitor residents to ensure targeted discharge, education and home care needs are met. Will assist with routine admissions and discharges as directed.

    Responsibilities

    Compliance:

     

    • Initiates the development and implementation of patient care standards, policies and procedures to be in compliance with regulatory requirements.

    • Develops, reviews and revises policies, procedures and other applicable operational documentation to ensure regulatory agency standards are met.
    •  
    • Ensures policies and procedures are adhered to by nursing staff. Monitors and maintains compliance with third party agency regulations.

     

    Documentation:

     

    • Completes and facilitates the completion of required documentation, i.e., admission assessments, risk assessments, discharge teaching, discharge paperwork and BPCI documentation.

    Employee Development:

     

    -Provides leadership and direction to personnel in accordance with organizational/ILS/RHCF goals and

    mission.

     

    -Maintains the highest level of customer service.

     

    -Ensures effective communication with the client/resident, family, and interdisciplinary team.

     

    -Informs Director of personnel activities and problems on a timely basis.

     

    -Counsels employees, taking appropriate educational and/or disciplinary action inclusive of termination,

    as necessary.

     

    -Interprets policies/procedures/regulations for the staff.

     

    -Fosters problem-solving abilities to staff.

     

    -Supports facility management and staff by facilitating change in a positive manner.

     

    -Ensures a safe and secure environment for clients/residents, staff, and visitors.

     

    -Shares current knowledge and up-to-date clinical expertise with staff through formal and informal

    education.

     

    -Takes responsibility for own education and stays clinically current.

     

    -Promotes a cohesive, productive supportive relationship between Director and self.

     

    -Effectively utilizes the counseling process with employees including significant issues after being

    reviewed with the Director.

     

    • Analyzes the problems, plans resolution strategies, implement plans and evaluates outcomes.

     

    • Under the director of Director, the Nurse Manager utilizes management and administrative skills to accomplish corporate, department and facility goals.

    • Acts as the clinical liaison to other Loretto facilities and programs regarding client/resident needs, etc. Case manages complex patient’s situation in collaboration with other members of interdisciplinary team.

    • Performs other duties as may be required.

     

    Resident Care:

     

    -Supervises the clinical care planning process to ensure the coordination and delivery of quality patient

    care.

     

    -Oversees the collaboration process/case management required to meet the critical needs of each

    client/resident.

     

    -Review all admission orders for accuracy.

    -Utilizes best practice guidelines.

     

    -Manages the delivery of patient care, maximizing quality and efficiency.

     

    -Effectively communicate between shift changes or triggers in resident/patient condition.

     

    • Provide assessment for resident/patient upon admission and periodically as regulations or changes in condition requires.
    •  
    • Obtain and implement physician orders. Ensures documentation is current and accurate.
    •  
    • Provide education and instruction to resident/patients, families and care givers as required.

    Interdisciplinary Team Process:

     

    -Collaborates with the Interdisciplinary Care Plan Team to:

                -Participate in family conferences to discuss the resident care plan

                -Keep the family informed of any resident changes

                -Facilitate discharge planning

                -Communicate any significant clinical changes in the resident that would alter the care plan

                -Identify the need for health teaching, teach nursing care issues to the resident and/or family members and

    verify/document understanding in a timely manner

     

    BPCI:

     

    • Ensures the IDCP team is aware the patient is under the Bundle Program.
    •  
    • After review of the patient, medical record and length of stay for the Bundle DRG ensures the appropriate discharge date is set and within length of stay goal and the IDCP team is aware.
    •  
    • Provides clinical assessment and oversight to identify clinical changes in an effort to avoid re-admission to the hospital.
    •  
    • Assure all paperwork is completed timely for the Bundle Program.

     

    • Complete discharge training/education for patients/families under the Bundle Program.
    •  
    • Work closely with the IDCP team to assure all equipment needs for safe discharge are in place.
    •  
    • Maintains and updates binders on rehab units up to date so all staff are aware of the Bundle patient and goals for discharge.
    •  
    • Provides weekly updates to nurse navigators and case managers as requested.
    •  
    • Maintains tracking log sheet for all resident’s covered under the BPCI program.
    •  
    • QA: Participates and conducts Quality Assurance audits as requested.

    Qualifications

    Qualifications (Degree, Certification, Experience)

    Must be a graduate of an accredited school of nursing, college or university for Registered Professional  

    Nurses.

     

    -Must possess a current NYS license and registration as an RN.

     

    -A minimum of 1-2 years of recent acute/long-term care management preferred.

    -Experience in team structured environment.

     

    -Proven assessment skills.

     

    -Proficiency in the care planning process.

     

    -Strong written and oral communication skills.
