  • Overview

    Provide comprehensive social work services to ALP/EL /SNF residents/clients. This includes, but is not limited to psychosocial evaluation, assessment for admission, patient's rights and advocacy, and social casework, group work and family counseling as appropriate. Key member of the interdisciplinary care team. Assure maximum quality of life for assigned residents/clients.

    Responsibilities

    Maintain accurate, complete, and timely MDS, psychological assessments, care plans (initially, quarterly, annually, significant change). Maintain accurate, complete and timely social work documentation on new admissions, including social  history, advance directive tracking sheet, family information, funeral arrangements, authorization to open mail, specialized unit agreements, progress notes. Counsel and instruct paraprofessional staff in psycho-social and communications issues and assists with difficult family/care-giver conflicts.  Provide social work interventions for residents and families in time of crisis, deteriorating health (physical and mental), dealing with disabilities, deterioration of independence and end of life, etc. Serve as advocate for resident/client in absence of caregiver/family or if resident is unable to do so.  Coordinate/facilitate initial, annual and prn resident and family meetings. Assure communication with family members not in attendance.  Assess resident financial situation, referring to finance department as necessary (i.e.:  Medicaid application). Provide financial counseling as required. Review significant events/occurrences as related to family, finance, and psychosocial behavioral changes with interdisciplinary team.

    Qualifications

    Bachelor’s degree of Social Work degree from an accredited program. Certified by the NYS Education Department as a Social Worker. Minimum of 2 years of professional experience with geriatric care, long term health care, or community services to the aging, in a direct service capacity.  Proven assessment skills Demonstrated ability to work in a team setting.  Thorough knowledge of regulatory requirements of DOH/DSS and all pertinent agencies.  Computer competency in MS Office suite or equivalent
