  • Overview

    The Admission Discharge Nurse (A.D.N.) assesses, plans, implements, and evaluates the effectiveness of nursing care delivered to the patients. S/he adheres to the established policy and procedures and the systemic clinical changes. The A.D.N. will be responsible for overseeing and completing the required assessments and documentation for the effective management of residents covered under the Bundled Payments for Care Improvement (BPCI) and will monitor residents to ensure targeted discharge, education and home care needs are met. Handles routine admissions and discharges.

    Responsibilities

    Resident Care:

    • Supervises the clinical care planning process to ensure the coordination and delivery of quality patient care.
    • Oversees the collaboration process to meet the critical needs of each client/resident.
    • Reviews all admission orders for accuracy.
    • Utilizes best practice guidelines.
    • Manages the delivery of patient care, maximizing quality and efficiency.
    • Effectively communicates between shift changes or triggers in resident/patient condition.
    • Provides assessment for resident/patient upon admission and periodically as regulations or changes in condition requires.
    • Obtains and implements physician orders. Ensures documentation is current and accurate.
    • Provides education and instruction to resident/patients, families and care givers as required.
    • Analyzes problems, plans resolution strategies, implement plans and evaluates outcomes.
    • Assists with the delivery of care, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated.

    Qualifications

    • Current NYS license and registration as an RN in good standing required.
