    The Home Care RN will participate with the planning, organizing, implementing and evaluating of Home Care services, as well as provide direct nursing care to the participant in their home.

    Responsibilities

    Top Responsibilities:

    Provides quality care through assessment, development, implementation and evaluation of individualized participant care plans.

    Teaches and counsels participants and caregivers as needed regarding maintenance of health and prevention of illness or complications.

    Delegates tasks appropriately and supervises, instructs and guides nursing and paraprofessional staff in providing high quality nursing services.

    Contributes as a member of the PACE Interdisciplinary Team. Acts as a liaison between the participant, the family/care givers, the home care supervisor and the PACE Interdisciplinary Team.

    Provides leadership among co-workers.

    Documents in an appropriate, thorough and timely manner and in accordance with Loretto Geriatric Center Licensed Home Care Agency’s medical records policies and procedures.

    Qualifications

    • Degree from an accredited school of nursing.
    • Current NYS license and registration as a registered professional nurse required.
    • Minimum of one year recent experience as a RN in an acute care or long term care setting.
    • Must possess a valid driver’s license without restrictions and a reliable, insured vehicle.
