  • Overview

    In conjunction with the Medical Director and a collaborating physician, the Nurse Practitioner (NP) provides comprehensive primary care to clients involved in the PACE program. May also make home visits and rounds on residents in the hospital and/or in a nursing home. Is responsible for acute, episodic and chronic disease management, which often includes but is not limited to, the prescribing of medications and therapeutics, ordering of diagnostics, interpretation of results and writing referrals to specialists as appropriate within the approved provider network. No weekends, but may be the provider On Call. Per Job Description.

    Responsibilities

    Fully documents pertinent - patient information in participant's record. Utilizes standard precautions appropriately. Performs all job activities in ways that convey respect and dignity for the individual and a responsiveness to other’s needs. Input data in PACEkeeper in a timely fashion, accurately reflect care provided, and in accordance with regulatory requirements. Demonstrates competence in disease management and geriatric principles.

    Qualifications

    Current NYS license as a Registered Nurse. Certification as a Nurse Practitioner in NYS. National certification as an ANP or GNP. Master of Science Degree in Nursing or its direct equivalent preferred. Two or three years’ experience in clinical nursing.  Ability to be very self-directed and utilize problem-solving and conflict resolution skills. Frequent exposure to patient care areas and associated hazards. Physical demands including lifting, positioning, pushing, and occasionally transferring patients, supplies or equipment. Excellent interpersonal and communication skills in order to collaborate with patients, families, physicians, staff and community agencies.
