Seeking RN for a leadership position in assisted living for the Loretto Housing division. Assisted living, Home Care or LTC experience preferred. Training available for acute care nurses wanting to transition to home and community based nursing. Responsible to independently case manage Housing residents living at our Adult Care facilities as they transition between site to hospital to rehab and home while ensuring supervision of the plan of care, accurate and complete documentation, medications and DME. Goal of this position is timely care management and coordination of care to decrease hospitalizations and or rehab, thus improving the resident’s care in the home and quality of life. Additional responsibilities include assessment and reassessment of the assisted living program, maximization of revenue by timely submission of DOH mandated UAS.
Current NYS license and registration as an RN in good standing required.
Graduate of an accredited school of nursing. BSN or BS in related field preferred.
Minimum (2) years community health nursing experience preferred.
Proven patient assessment and care planning skills.
Current knowledge of best practice nursing standards and care.
Knowledge of adult care facilities and applicable regulations desired.
Outstanding interpersonal skills including the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
Proven conflict resolution skills.
Proficient in MS Office programs and able to learn new software rapidly.
An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements
