  • Overview

    Coordinate the ordering an delivery of resident medication. Responsible for ensuring the availability of resident medications according to physician orders. Performs audits on all prescriptions to ensure the availability and accuracy of inventory.

    Responsibilities

    • Ensures resident medications and supplies are available and in stock. Ensures that residents/clients are provided therapeutic and rehabilitative measures to maximize independence.
    • Orders and reorders medications from pharmacies per physician orders
    • Transcribes physician orders.
    • Faxes orders to pharmacies and/or orders medications electronically.
    • Monitors all documentation pertaining to medications for accuracy and completeness.  
    • Documents narcotics in the Narcotic Sign-In book.
    • May document resident issues and conditions in the medical record.
    • Checks deliveries from pharmacy for accuracy against physician orders and follows up as needed.
    • Follows up promptly with pharmacies if prescription delivery is delayed.
    • Escalates delivery issues to Clinical Supervisor.
    • Responsible for proper storage and security of medications, including narcotics, in accordance with established policies, procedures and state/federal guidelines.
    • Assists in maintaining a clean and organized medication cart and supply area.
    • Assists with Quality Assurance/Performance Improvement (QAPI) activities on a monthly and quarterly basis.
    • Demonstrates awareness of proper infection control practices.
    • May respond to resident concerns and requests.
    • Communicates medication information accurately and factually. Seeks clarification from RNs and other clinical or ALP/EL leadership when needed.
    • Communicates information pertaining to medication at shift change.
    • Removes discontinued narcotics and ensures compliance with narcotic regulations.
    • May, under the direction of Clinical Supervisor, provide residents with medication and treatment assistance and services according to the health care provider's orders and policies and procedures.
    • Has awareness of the Five Rights of Medication Administration (Right resident, medication, route, time and dose) when organizing work.
    • Assists in the supervision of residents who self-administrator medication while following policies, procedures and regulatory guidelines.
    • Assists to report any changes in residents' physical condition and/or behaviors
    • Maintains resident records regarding medication receipts from pharmacy.

     

    Qualifications

    • Prior experience as a home health aide or working with older adults in an adult care setting.
    • Certification as a Home Health Aide desired.
    • High school diploma or equivalent required.
    • Familiarity with medical transcriptions.
    • Working knowledge of applicable regulations for medication management and senior housing.
    • Strong attention to detail and follow up skills.
    • Excellent interpersonal and communication skills including the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
    • Professional email etiquette.
    • Strong reading comprehension ability.
    • Proficient in MS Office programs and able to learn new software rapidly

     

    An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
