The Home Care Coordinator RN assists with care coordination of PACE participants; acts as the liaison for the interdisciplinary team between the contracted home care agency, hospital, and other facilities. This is a day shift position that does not require weekends or holidays.

    Responsibilities

    Top Five Responsibilities Include:

    • Coordinating Discharge planning.
    • Coordinating care planning with other facilities in conjunction with interdisciplinary team.
    • Assists in evaluation for potential new enrollees.
    • Communication liaison between hospitals/facilities.
    • Provides leadership among coworkers.

    Qualifications

    • Currently licensed as a Registered Nurse in New York State.
    • 2 years' experience in nursing; preferably working with geriatric population.
