The Home Care RN is responsible for providing the highest quality of care in their home and ensuring safety at all times according to accepted nursing practice standards.
Top Five Responsibilities:
Provides quality care through assessment, development, implementation and evaluation of individualized participant care plans.
Teaches and counsels participants and caregivers as needed regarding maintenance of health and prevention of illness or complications.
Delegates tasks appropriately and supervises, instructs and guides nursing and paraprofessional staff in providing high quality nursing services.
Contributes as a member of the PACE Interdisciplinary Team. Acts as a liaison between the participant, the family/care givers, the home care supervisor and the PACE Interdisciplinary Team.
Provides leadership among co-workers
Must have current NYS RN license and valid NYS driver's license, and vehicle to travel Onondaga County. Minimum one year nursing experience in acute care, LTC, or Home Care.
