**Sign-On Bonus**
Under the supervision of the Home Care Supervisor, the LPN is responsible for providing direct physical nursing care to the patient. This individual is responsible for providing the highest quality of patient care in the home and ensuring patient safety at all times according to accepted nursing practice standards.
Must have current NYS LPN license, and valid driver's license and vehicle to travel Onondaga County. |
Minimum one year experience in acute or LTC nursing. Prefer PACE and/or home care experience. Hours and community assignments based on department needs (typically 5P-9P, 20 hours per week).
