Overview

Seeking RN to complete clinical case management and QA activities for the Loretto Housing Division under the supervision of the Director of QA and Director of Clinical Services. Responsibilities include part time QA activities and part time case management activities.Training in the UAS and PRI/Screen certification a plus. One year experience in long term care and or home care required. BS preferred.

Responsibilities

  • Assists in the planning, organization and direction of Quality Assurance program and related activities for the Housing division.
  • Assists in the coordination and completion of audits performed within the QA program and prepares reports as required.
  • Performs QA audits for the LHSCSA and the clinical portion of the EL/ALR/AH programs.
  • Establishes and maintains tracking systems of identified problems/indicators.
  • Responsible for report cards for the LHCSA and the EL/AL/AH as directed.
  • Assists department heads and staff in the evaluation of programs and assists to effect changes to improve programs and ensure compliance with regulatory requirements.
  • Handles case management responsibilities by covering for other RNs in the Home Care agency for periodic assessment, evaluation and case management needs of residents of the Assisted Living Program.
  • Completes periodic assessments and reevaluations of residents for the Assisted Living Program under the direction of the Director of Clinical Services.
  • May attend and actively participate in utilization review meetings and provide appropriate follow-up to all disciplines within the Loretto system.
  • Ensures that all relevant and required documentation is complete, accurate and timely.
  • Keeps current in quality management practices and case management skills, and in emerging and best practices.

Qualifications

  • Bachelors’ degree from an accredited college.
  • 1-2 years’ full time nursing experience in home care, adult care, or long term/acute elder care.
  • Current NYS licensed registered nursing license and registration.
  • Experience with and knowledge of quality assurance activities and process improvement techniques.
  • Knowledge of DSS and DOH regulations which apply to Enriched Living, Assisted Living, Assisted Living Residence and Licensed Home Care (LHCSA) preferred.
  • Current knowledge of best practice nursing standards and care.
  • Outstanding assessment skills.
  • Ability to work independently and as part of a team.
  • Outstanding interpersonal and communication skills including the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
  • Effective problem solving and conflict resolution skills.
  • Must possess valid driver's license and a reliable vehicle with insurance.
  • Proficient with MS Office and other applications.

 

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
